You will be required to enter an invitation code, which is the last four digits of your social security number. You must have your SSN on file with Cedar Valley Medical Specialists to verify access to your medical records.

Signup is easy through your Google, Yahoo, Facebook, or Windows ID (Hotmail) account. If you do not have one of these secure accounts, creating one is fast and easy.
